My plan is to start a turn team to help prevent pressure ulcers/ prevent worsening ulcers.I have plans to implement a turn team protocol. The protocol will be important in supporting the patients with the need to prevent him from acquiring hospital infections. Further, it will prevent the chances of the patient experiencing muscle strains as a result of being in a particular position for long. There will be a need to develop a chart that the CNA and the nurse will need to fill out. The chart will need to detail the name of the nurse, and the patient as well as the condition of the patient.  It will also need to detail the last time the patient was turned. Thus, the strategy will seek to guide the nurse and the CNA on the appropriate procedure which they need to undertake in the process of turning the patient and improving his health outcomes.      After turning the patient, the patient has been turned, the health professionals will need to document the activity. They will also need to include the position on which the patient was turned. The action will help to guide them appropriately on the next time they need to turn the patient. It will also help to prevent the formation of ulcers or the worsening of the current ulcers which the patient has. The protocol is established with the aim of preventing the need to hire more personnel to engage in the exercise. Each nurse and CNA will be in charge of turning there own patient and document the time its done and side the patient is now on as well as what is being uses ( pillows or wedges under the patient.    The turning protocol will help to improve the mobility of the patient. It also improves the chances of the patient to heal from other health problems as well. Where the plan achieves its purpose of preventing the development of other health problems, the amount of money saved in the process is significant. Discuss how forming the in house team will save money - This is a free proposal only a new procedure would have to be put in place - The unit will not need to hire new employees and the current employees with be responsible from following the chart and documenting accordingly.  Checking the patient regularly will allow any skin changes to be caught early on. Please discuss how this will also help the unit save by prevent skin break down.
